Thank you for your interest in our dogs. We are glad that you have considered adopting a malamute from Second Chance Alaskan Malamute Program, Inc. Please fill out the following questionnaire. We will use your answers as a starting point to work with you to determine whether or not a Malamute is really what best fits your lifestyle. If so, we will then help determine which Malamute best fits your expectations and lifestyle.

This is an adoption application-it is not an agreement to adopt a dog. SCAMP, Inc. reserves the right to refuse adoption to anyone.
